Rose Hill Center provides psychiatric rehabilitation services that are effective in helping people successfully achieve and maintain their highest level of function and independence.
To be considered for admission, individuals must be:
- Primarily diagnosed with schizophrenia, schizoaffective disorder, bi-polar disorder
or other major mental illness
- At least 18 years old
- Not actively suicidal or harmful to others
- Not currently verbally or physically aggressive
- Not currently using drugs or alcohol
- Independent with personal care activities
- Willing to participate in a structured daily program as directed by
an individualized treatment plan
- Willing to take prescribed medications
Referrals to Rose Hill may be initiated by a:
- Psychiatrist
- Mental health professional
- Individual/applicant
- Family member
Admission process is as follows:
Step 1: Phone the Rose Hill Admissions Office at 248-634-5530
Step 2: Download and print the application forms.
- Physician Referral Form - This form
is to be completed by the current attending psychiatrist and should be
sent along with supporting documents such as lab results, psychiatric
evaluations, psychosocial, progress notes, discharge summaries, etc.
NOTE: In order to facilitate the transfer of confidential information to Rose Hill
Center, this Release Form should be completed, signed and forwarded to the physician/psychiatrist.
- Resident Application Form - This form is to
be completed by the applicant.
- Financial Form - This form is to be completed
by the financially responsible party.
- Benefit/Contact Information Form - This form is to be completed by applicant and/or applicant's family. Copies of the documents listed under the heading "Necessary for Admission" on page 2 of this form must be included.

Step 4: Participate in a face-to-face
interview
Upon receipt of the completed application and necessary clinical documentation, the Director of Admissions will contact the applicant and make arrangements for a pre-admission interview.
A final word ...
The Rose Hill Admissions Committee evaluates each applicant for potential
admission. At the completion of the process, the Director of Admissions
will inform the individual of his/her acceptance status.
