Transitional Living:
One step closer to achieving full potential
Rose Hill Center's two transitional programs, Comprehensive Transitional Living (CTL) and Modified Transitional Living (MTL), help bridge the gap to community living. These programs are designed specifically for graduates of the Rose Hill Residential Rehabilitation Program who have achieved basic levels of independence, but who need support and guidance before moving back into the community.
While
enrolled in either of the transitional living programs, participants share a two-bedroom townhouse with one other person. The seven townhouses are located in the Helen L. DeRoy Village near the residential buildings on Rose Hill's campus, thus allowing individuals to experience independent living in a secure, supportive environment.
The goal of both the Comprehensive and Modified transitional programs is to enable clients to achieve and maintain their highest level of independent functioning. This is accomplished by:
- Promoting insight into mental illness and its management
- Assisting participants with medication management and compliance
- Providing an opportunity for participants to acquire and practice the skills needed to maintain a household, including grocery shopping, meal preparation, housekeeping, laundry, budgeting, and bill paying
- Assessing the individual’s capacity for successful independent living
- Providing and/or promoting activities to strengthen social skills
- Promoting a healthy lifestyle
- Requiring that participants structure each day, and engage in meaningful activity such as paid employment in the community, school attendance, or volunteer work for at least 20 hours per week
Rose Hill’s professional mental health team, made up of a case manager, psychiatrist, psychiatric nurse, social worker, and support staff, provides the following core services for all Transitional Living clients:
- Assistance with ordering medication, setting up weekly trays, scheduling lab work
- 24-hour on-call crisis and emergency support services
- Annual treatment plan with quarterly reviews
- Assistance in managing benefits and insurance; liaison to social service agencies
- Weekly support groups and activities
- Use of the facilities at the Education and Therapy Center on a scheduled basis
- Social events at Rose Hill Center
- Local transportation to community activities/events on weekends and evenings
Both programs also provide the following supportive services, although the frequency of supportive services provided in the two programs will vary:
- Case management and supportive counseling visits
- On-site psychiatric appointments for medication monitoring
- Assessment of independent living environment
- Assessment of mental health status
Comprehensive Transitional Living (CTL)
Comprehensive Transitional Living participants must be graduates of the Residential Rehabilitation Program (although this requirement may be waived by Rose Hill’s president.) They must also be recommended by the Rose Hill clinical team and approved by the president. Supportive services are offered with greater frequency and intensity in the Comprehensive Transitional Living program. In addition, CTL services include local transportation which can be scheduled for employment, volunteer service, school, appointments and shopping during the daytime, and one meal per day at the Kelly Community Center.
Total cost for the Comprehensive Transitional Living program is $100 per day.
Modified Transitional Living (MTL)
In order to qualify for the Modified Transitional Living program, individuals must have already participated in the Comprehensive program (although this requirement may be waived by Rose Hill’s president.) In addition, they must be recommended by the Rose Hill clinical team and approved by the president. Supportive services are provided less frequently than in CTL. Meals and daytime transportation for employment, volunteer service, school, appointments or shopping are not included in the daily MTL rate, however, these services may be purchased separately.
Total cost for the Modified Transitional Living program as outlined above is $60 per day.
