Community Support:
Attaining -- and maintaining -- independence

Rose Hill Center's Community Support Program provides case management services on an ongoing basis to individuals who are residing independently in a community near Rose Hill.

These individuals are responsible for structuring their week with meaningful daily activities for
a minimum of 20 hours per week. Activities may include gainful employment, further education, or volunteerism. In order to establish a healthy, well-rounded lifestyle, Community Support participants are also expected to demonstrate the ability to manage their medications, structure their leisure time, and solve the problems of everyday living . 

In turn, Rose Hill supports their independence by offering the following fee-based services:

  • Individualized treatment plans and quarterly reviews
  • Regularly scheduled visits with a psychiatrist
  • Participation in support groups and social events
  • Regularly scheduled visits with a case manager
  • Continued medication education and supervision
  • Continued support in regard to the activities of daily living
  • Assistance with vocational programs, day programs, job acquisition and retention
  • Therapeutic drug blood-level monitoring (if applicable and per insurance coverage)
  • On-call, 24-hour emergency supports
  • Pre-arranged transportation
